Metatarsalgia is pain in the ball of the foot, where the metatarsal heads take load with every step — worst in thin-soled shoes and high heels.
A flat, unsupported forefoot concentrates load on the exact spot that already hurts.
These shoes are selected with a cushioned forefoot, a metatarsal support in the footbed, and in many styles a rocker sole that reduces how hard the ball of the foot has to work. Load through the forefoot is not something you can train away, which is why the load path matters: a sole that stays flat under the ball of the foot delivers the full step straight through the one area that is already complaining, while a slight rocker lets the foot roll forward instead of pushing down.
Low and cushioned is the safer direction. If you do wear heels occasionally, keep them low, wide-based and short-wearing.
A raised pad under the ball of the foot in the footbed. It spreads load across the metatarsal heads instead of letting one small area take all of it.
Begin with forefoot cushioning and a wide toe box. See Ténis de caminhada and Dor no pé for the roomier fits.
If the pain in the ball of your foot is persistent or worsening, please see a podiatrist or your doctor. Footwear is one part of everyday foot comfort — it is not a treatment.
